       Proceed to High School Department.

• One (1) Valid ID

1. Go to Guidance Office

• Bring Form 138 (Report Card) Original or Photocopy with at least 85% average
• Get Application Form
• Bring 2 pcs. 1×1 ID picture (white background)

2. Cashier

       Pay the Testing Fee (PHP 100.00)

3. Got to Guidance Office

• Present the original receipt of the testing fee
• Submit the duly accomplished application form
• Get your exam schedule

4. Bring Pencil with eraser
